  • Department Leadership: Direct, develop, and evaluate the facility’s social services program to meet the psychosocial needs of all residents.

  • Assessments & Care Planning: Complete comprehensive social history evaluations, initial assessments, and progress notes; actively participate in interdisciplinary care plan meetings.

  • Discharge Planning: Coordinate safe, seamless transitions and discharge planning back to the community, arranging home health, durable medical equipment (DME), and community resources.

  • Resident & Family Advocacy: Act as the primary liaison and advocate for residents and families, navigating financial, emotional, and placement needs.

  • Regulatory Compliance: Ensure all social service workflows comply with federal, state, and local long-term care regulations.

Requirements & Qualifications
  • Education: Master of Social Work (MSW) or Bachelor of Social Work (BSW) from an accredited institution.

  • Licensure: Current, unencumbered license as a Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) in the State of Ohio (Required).

  • Experience: Minimum of 2 years of social work experience in a Skilled Nursing Facility (SNF), Long-Term Care (LTC), or sub-acute rehabilitation setting (Highly Preferred).

  • Skills: Exceptional communication, conflict-resolution, crisis intervention, and organizational skills. Proficiency with electronic medical records (EMR) software.
