Job description

Travel, Social Worker - LCSW

Location: Camden, ME

Shift: 5x8 Days, 08 am to 4 pm

Duration: 14 Weeks

Guaranteed Hours: 40

Hourly Pay: $44/hr

Key Responsibilities

  • Conduct initial and ongoing psychosocial assessments to identify residents' emotional, social, and environmental needs
  • Collaborate with interdisciplinary care teams to develop and implement individualized care plans
  • Coordinate complex discharge planning, including community referrals, home health services, and durable medical equipment
  • Provide short-term counseling and crisis intervention to residents and families
  • Advocate for resident rights and assist with advance directives and power of attorney matters
  • Support residents and families with financial resource guidance, including Medicaid and insurance navigation
  • Maintain accurate, timely documentation in compliance with state and federal regulations
Requirements:
  • Licensure: Active Social Work license in the State of Maine (LSW, LMSW, or LCSW required)
  • Education: Bachelor's degree in Social Work (BSW) required; Master's degree (MSW) preferred
