Job description

Our client based in Somerset is seeking an experienced Adult Social Worker to join their Acute Discharge Team, working on-site within a hospital setting to support safe and timely discharges.
This is a fast-paced role focused on hospital discharge in line with the Intermediate Care Process, where you will deliver person-centred, strengths-based interventions to support individuals transitioning from acute hospital settings back into the community.
You will play a key role in facilitating effective discharge planning, ensuring individuals receive the right support at the right time, while promoting independence and managing risk. Working within a multi-disciplinary team, you will complete assessments, apply relevant legislation, and contribute to safe discharge outcomes.
Key Responsibilities:

Core duties include:

  • Managing a caseload of adults requiring support with hospital discharge from acute settings.
  • Supporting safe and timely discharges in line with the Intermediate Care Process.
  • Undertaking Care Act 2014 assessments, discharge planning, and strengths-based support plans.
  • Completing Mental Capacity Assessments and contributing to Best Interests decisions where required.
  • Applying a strong working knowledge of the Mental Capacity Act (MCA) in discharge planning.
  • Working closely with individuals, families, and carers to achieve person-centred discharge outcomes.
  • Collaborating effectively within multi-disciplinary teams (MDT) including healthcare professionals, discharge coordinators, and community services.
  • Identifying appropriate post-discharge support, including reablement, care packages, and community-based services.
  • Assessing and managing risk to ensure safe and sustainable discharges.
  • Supporting individuals transitioning from acute hospital care into community or residential settings.
  • Maintaining accurate, timely, and high-quality case records in line with statutory requirements.
  • Applying a strengths-based and outcome-focused approach to all interventions.
  • Operating effectively in a high-pressure hospital environment, prioritising urgent discharge work.

Qualifications & Experience:

Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW

Registration: Must be registered with Social Work England

Experience:

  • Minimum 2 years' post-qualified experience, including completion of ASYE
  • Strong experience within Adult Social Care
  • Essential experience within acute hospital or discharge settings
  • Experience working within a fast-paced environment
  • Experience completing:
    o Care Act assessments and discharge planning
    o Mental Capacity Assessments
    o Best Interests decisions
  • Experience of multi-disciplinary working
  • Experience supporting hospital discharges and community transitions
