Qualifications & Experience Requirements for Licensed Social Worker:

  • Previous Experience as a Social Worker preferred
  • Social Worker License
Qualifications for Licensed Social Worker:
  • Psychosocial assessments/BIMS/PHQ9/Trauma Induced Care
  • Attend Care plans/meetings
  • MOLST review
  • Must have Knowledge of HCPOA/Advanced Directives, PASSAR/ID screens
  • Work with the Interdisciplinary team with regards to discharge planning, MDS completion, formulate individualized care plans
  • Provide resources PRN
  • Advocate for our Seniors Aging in Place
  • Attend meetings as appropriate
