Job description

Social Worker

This role delivers comprehensive social work services within the regional area, supporting patients and families through assessment, care planning, and coordination of resources. Responsibilities include conducting psychosocial evaluations, developing individualized intervention plans, providing counseling, facilitating access to community supports, and collaborating closely with interdisciplinary teams to ensure continuity of care. The position requires strong communication skills, clinical judgment, and the ability to manage a varied caseload. A Bachelor’s or Master’s degree in Social Work and appropriate licensure are required.
