Job description

Social Service Director

At Alden Long Grove Rehabilitation and Health Care Center, we are seeking a dedicated and compassionate Social Service Director to lead our Social Service Department. As a key member of our team, you will be responsible for planning, developing, organizing, and overseeing the overall operation of the department, ensuring that our residents receive exceptional social services.

Job Summary:

As a Social Service Director, you will be responsible for performing assigned social work duties and responsibilities within the facility. This includes assessing and documenting residents' social, emotional, and mental health needs, developing individualized care plans, and collaborating with the Interdisciplinary Team to ensure high-quality care. You will also provide supportive counseling and behavioral intervention programming, act as a resident advocate, and supervise, educate, counsel, and discipline social service staff.

Responsibilities:

  • Plan, develop, organize, oversee, and run the overall operation of the Social Service Department

  • Assess and document residents' social, emotional, and mental health needs

  • Develop individualized care plans and collaborate with the Interdisciplinary Team

  • Provide supportive counseling and behavioral intervention programming

  • Act as a resident advocate and liaison between residents, families, and community agencies

  • Supervise, educate, counsel, and discipline social service staff

  • Complete initial and ongoing assessments, MDS, CAAs, and care planning initiatives

  • Enter and maintain accurate records and documentation

  • Participate in quality improvement initiatives and safety committees

  • Perform other related duties as assigned

Requirements:

  • Bachelor's Degree in Human Services field (e.g., Social Work, Psychology, Counseling, Sociology, or related field)

  • One year of experience in social service/case management in the healthcare field

  • Current knowledge of Federal and State Long-Term Care (LTC) Social Service regulations

  • Ability to work with and understand the needs of residents, families, and staff

  • Excellent communication and interpersonal skills

  • Ability to make independent decisions and handle confidential data with discretion

  • Leadership ability and willingness to work harmoniously with other personnel

  • Ability to accept and utilize professional supervision, consultation, and in-service training/educational opportunities

Physical Requirements:

  • Ability to move intermittently throughout the workday

  • Possess sight/hearing senses or use prosthetics to function adequately

  • Ability to function independently and work effectively with residents, family members, personnel, and support agencies

  • Ability to cope with mental and emotional stress and relate to and work with physically and mentally ill residents

  • Ability to participate in physical crisis prevention techniques and lift equipment and supplies

Salary: $65,000 - $65,000 ANNUALLY
