) Social Services Coordinator
Facility: Highland Chateau Health + Rehabilitation
Location: St. Paul, Minnesota
Department: Social Services
Reports To: Administrator
Employment Type: Full-Time
Position Summary
The Social Services Coordinator is responsible for coordinating and providing psychosocial services to residents and their families in accordance with federal and Minnesota state regulations. This role supports resident well-being, discharge planning, care conferences, and advocacy while ensuring compliance with skilled nursing facility (SNF) requirements.
Essential Duties and Responsibilities
  • Conduct psychosocial assessments and ongoing evaluations of residents' emotional, social, and environmental needs
  • Coordinate and participate in care plan meetings, family meetings, and interdisciplinary team discussions
  • Facilitate discharge planning, including coordination with community resources, hospitals, and outside agencies
  • Assist residents and families with adjustment to illness, disability, or long-term care placement
  • Advocate for resident rights and support grievance resolution
  • Provide counseling and crisis intervention as needed
  • Maintain accurate, timely, and compliant documentation in the medical record
  • Ensure compliance with federal, state, and local regulations, including CMS and Minnesota Department of Health requirements
  • Collaborate closely with nursing, therapy, admissions, and business office teams
  • Support advance directives, guardianship processes, and resident/family education
Required Qualifications
  • Bachelor's degree in Social Work, Human Services, Psychology, or a related field
  • Minimum of 1 year of experience in a healthcare or long-term care setting preferred
  • Knowledge of federal and Minnesota SNF regulations related to social services
  • Strong communication, documentation, and organizational skills
  • Ability to work effectively with residents, families, and interdisciplinary teams
Preferred Qualifications
  • Licensed Social Worker (LSW or higher) in Minnesota
  • Experience in skilled nursing or post-acute care settings
  • Familiarity with discharge planning and community-based resources
Physical and Work Requirements
  • Ability to sit, stand, and walk throughout the facility
  • Occasional lifting of up to 25 pounds
  • Ability to manage multiple priorities in a fast-paced healthcare environment

This position is essential to maintaining regulatory compliance, supporting resident psychosocial needs, facilitating safe and timely discharges, and enhancing resident and family satisfaction. The Social Services Coordinator plays a critical role in interdisciplinary care planning and quality outcomes.
