Facility Medical Director & Supervising Physician - Geriatric Care
SCHEDULE:

This organization is a leading provider of integrated medical services, caring for individuals from post-hospitalization to home in various settings.  They bring quality medical care to the geriatric population, as well as people with complex conditions, special needs, and intellectual and/or developmental disabilities.  Their providers make a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve.
POSITION DESCRIPTION: 
This organization is seeking a physician leader for a facility medical director and supervising physician role, which includes some patient care duties.
Physicians see patients in skilled nursing facilities and other long-term care settings.  Providers work autonomously and are remotely supported by a robust clinical and administrative team.
The duties of the role include, among other responsibilities:
- medical directorship(s) for SNFs and/or other long-term care facilities
- ensure engagement with facility leadership, other providers, patients, and families
- help identify growth opportunities with SNFs and other facilities in local market
- supervise nurse practitioners and physician assistants
- audit documentation for each supervised provider on a monthly basis
- provide high-quality complex geriatric care to patients
- collaborate with other healthcare providers as needed
- participate in the on-call schedule
- attend required trainings and meetings
The new physician leader will also be charged with keeping focus on quality outcomes such as:
- care path compliance
- productivity
- prevention of avoidable transfers
- decrease in polypharmacy
- costs of care
The practice uses an EHR on a mobile-based platform.
REQUIREMENTS:
- Indiana medical license
- DEA
- Board Certification, relevant field (Internal / Family Medicine, Geriatrics, etc.)
- 2+ years clinical leadership or supervising physician experience
- 5+ years clinical practice experience
- previous experience with geriatric primary care or a related field
