Job description

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Breast Care Coordinator is responsible for guiding patients through the continuum of breast health services within the hospital system, from community outreach and education through screening and diagnostic evaluation until a benign or malignant diagnosis is made. This role serves as a clinical and supportive liaison between patients, providers, imaging centers, oncology services, and community resources to ensure timely access to care, reduce barriers, and improve patient outcomes. The coordinator collaborates with multidisciplinary teams to facilitate seamless navigation, patient education, follow-up care, and quality improvement initiatives related to breast health. This position also provides cross-departmental support to ensure continuity of care and operational efficiency across screening and navigation programs.

Qualifications:
Required:

  • Bachelor’s degree in public health or related healthcare field
  • Minimum of 1 year of experience in community health, women’s health, breast imaging, care coordination, or patient navigation
  • Certified Navigator Breast Advocate (or obtained within 1 year of hire)
  • Knowledge of healthcare systems, scheduling processes, and community resources

Preferred:  

  • Experience within an accredited breast center or oncology program
  • Experience with community outreach initiatives and cancer screening programs
  • Familiarity with regulatory and accreditation standards related to breast cancer care
  • Bilingual skills preferred based on community needs

Scheduling Requirements 

  • Monday – Friday 
  • Day Shift 
  • Full-Time, 40 Hours Per Week

Essential Functions:

  • Coordinate and navigate patient care from breast cancer screening and abnormal findings through diagnostic workup and follow-up care
  • Serve as the primary point of contact for patients with abnormal breast imaging to ensure timely access to services and reduce barriers to care
  • Educate patients and families regarding breast health, diagnostic procedures, support services, and available community resources
  • Collaborate with radiology, surgery, oncology, pathology, primary care, genetics, and ancillary services to facilitate seamless multidisciplinary care coordination
  • Conduct community outreach and education initiatives focused on breast cancer awareness, prevention, screening, and early detection
  • Assist patients in overcoming social drivers of health barriers including transportation, insurance, financial concerns, language access, and appointment coordination
  • Maintain accurate documentation, patient tracking systems, and program metrics in accordance with hospital, accreditation, and regulatory standards
  • Participate in breast program quality improvement initiatives, tumor boards, accreditation activities, and performance reporting
  • Collaborate with marketing, physician liaisons, and community partners to promote breast health services and increase screening participation within the community
  • Provide emotional support and compassionate guidance to patients and families throughout the continuum of care
  • Support development and implementation of breast health navigation workflows, policies, and evidence-based practices across the health system
  • May perform other duties as assigned or directed by management
