Job description

Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities and hospitals.

Quality Management Coordinator is responsible for performing chart auditing and focus studies, assisting the Director of Quality Management with the analysis and dissemination of data, participating in Quality Improvement activities &  programs in hospice and organization, QAPI Committee and RCAs. Participates in CMS mandated Quality reporting HIS & CAHPS.  Maintaining current knowledge of regulatory bodies and standards related to Hospice delivery of services and educates staff regarding quality improvement, regulatory and accrediting body issues.

  • Licensed with current Registration to practice as Registered Professional Nurse in NYS is required
  • Baccalaureate Degree in Nursing required
  • Experience in Hospice or Quality Management/Performance Improvement
  • Knowledge of application of Statistical Processes Control Tools, Cops and NYS DOH Regulations
