Second Chance Marriage and Family, Inc. is actively seeking a passionate and skilled Licensed Clinical Social Worker (LCSW) to fill a full-time position primarily focused on hospice care. Work Environment

This position is primarily remote, allowing for flexible scheduling that accommodates the needs of both our clients and staff. Key Responsibilities
  • Conduct thorough biopsychosocial assessments to develop individualized treatment plans that reflect the diverse needs of each client.
  • Utilize various evidence-based behavioral therapies including solution-focused therapy, motivational interviewing, and problem-solving treatment to establish effective therapeutic relationships with clients.
  • Regularly monitor and document client progress towards treatment goals, using both in-person and remote methods to provide continuous support.
  • Collaborate closely with a multidisciplinary team, including doctors, psychologists, and hospice care staff, to ensure an integrated approach to client care that supports both physical and mental health needs.
  • Assist clients in navigating their emotional responses to illness and loss while providing education and resources related to end-of-life care and bereavement.
  • Administer psychotropic medications in coordination with medical providers, focusing on adherence, efficacy, and side effects to optimize client health outcomes.
Patient Population / Client Focus

This role primarily serves patients in a hospice setting, which often includes individuals facing terminal illnesses and their families. Qualifications
