Job Summary

Position Title: Community Outreach Liaison

Position Summary: Essen Health Care is seeking a motivated and community-focused Community Outreach Liaison to support outreach initiatives across the Bronx, Brooklyn, Queens, and surrounding communities. This role is responsible for building relationships within the community, promoting healthcare services, supporting patient engagement efforts, and increasing awareness of Essen Health Care programs and resources.

The ideal candidate is personable, organized, culturally aware, and passionate about improving access to healthcare for underserved populations.

  • Conduct community outreach activities to promote Essen Health Care services and programs
  • Build and maintain relationships with community organizations, local businesses, schools, senior centers, shelters, and other partners
  • Attend health fairs, community events, and outreach initiatives on behalf of Essen Health Care
  • Educate community members on available healthcare services, insurance resources, and wellness programs
  • Assist patients with scheduling appointments and connecting to appropriate departments
  • Support membership growth and patient retention initiatives
  • Maintain outreach logs, event tracking, and community engagement reports
  • Distribute marketing and educational materials within the community
  • Collaborate closely with operations, marketing, care management, and clinical teams
  • Represent Essen Health Care professionally and positively in all community interactions
QualificationsQualifications
  • Minimum High School Diploma or GED required
  • Associate's or Bachelor's degree preferred
  • Previous experience in community outreach, customer service, healthcare, social services, or patient engagement preferred
  • Strong interpersonal and communication skills
  • Ability to work independently and in a team environment
  • Bilingual skills strongly preferred (Spanish, Russian, Chinese, or other languages a plus)
  • Comfortable working in the field and traveling locally throughout NYC boroughs
  • Basic computer skills including Microsoft Office and email communication
