    • Job Summary:
      • The Community Health Worker (CHW) will support Community Wellness Outreach initiatives by enhancing care coordination, addressing social determinants of health (SDOH), closing care gaps, and improving patient care. The CHW will play a critical role in improving patient outcomes, reduce readmissions, and reduce health disparities across Calvert County.
    • Education:
      • High school diploma required.
    • Registration/Certification/Licensure:
      • Maryland Community Health Worker Certificate required. 
    • Experience:
      • 1+ years of relevant experience. 
    • Other Requirements:
      • Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements.
      • Serve as a liaison between patients, healthcare providers, and community resources to enhance patient access to care.
      • Accurately document patient interactions, referrals, and follow-ups.
      • Current CPR within 30 days of hire.
