Essential Services Core | SNAP

SNAP is seeking a qualified individual for a Community Health Worker II position to work in SNAP’s Essential Services Core. This position is full-time (40 hours per week), regular, and open to all qualified individuals.

The Community Health Worker II plays an important role in supporting SNAP’s mission by helping individuals and families access essential services that promote stability, health, and well-being. This role focuses on outreach, client engagement, service coordination, and transportation support to help community members overcome barriers and connect with the resources they need.

Full Salary Range: $20.11 – $31.49 per hour

Position Summary

Position Summary:   The Community Health Worker II plays a vital role in supporting individuals and families through proactive outreach, engagement and service coordination. This position focuses on delivering a broad range of supportive services, including life skills development, employment assistance, access to meals and groceries, mental and outpatient health service navigation, transportation support, case management and housing stability resources. This position requires strong communication skills, knowledge of community resources, and the ability to build trusting relationships with diverse populations.

Key ResponsibilitiesClient Engagement & Support

Develop trusting relationships with clients and their families while providing encouragement and guidance.

Work with clients to assess needs, develop service plans, and support progress toward goals.

Provide ongoing follow-up with clients through phone calls, home visits, and visits to community locations.

Assist clients with completing applications for services such as food benefits, SSI, TANF, GAU, and housing programs.

Serve as a client advocate and liaison between clients and service providers.

Service Coordination

Maintain knowledge of community resources available to clients and families.

Provide referrals to community agencies and coordinate services to support client stability.

Collaborate with medical providers, behavioral health providers, and community organizations to ensure coordinated care.

Transportation & Client Assistance

Safely transport clients to and from appointments using SNAP-owned vehicles.

Assist clients entering and exiting vehicles and provide support as needed.

Operate wheelchair ramps and securement devices when required.

Perform pre-trip safety inspections and accurately record vehicle mileage.

Report any vehicle concerns or transportation issues to a supervisor.

Documentation & Team Collaboration

Document client services and interactions in the client database within 24 hours of contact.

Maintain accurate records to support program tracking and reporting.

Attend staff meetings and required trainings.

Work collaboratively with coworkers and volunteers to promote a respectful and productive work environment.

Serve as an ambassador of SNAP’s mission and customer service culture.

Requirements:

                Education and Experience

  • Required:  HS/GED and at least three years of experience related to Community Health, patient/client support, transportation services or related work – OR –2 years’ experience AND at least 12 months of experience as a SNAP CHW I. Must have clean driving record and a valid driver’s license. Must be able to pass Department of Transportation drug testing and secure DOT Medical Clearance. Must be age 21 or over.
  • Preferred:  Strong preference for lived or professional experience related to low-income, underserved populations, with varied health economic and educational circumstances.   Bi-lingual speaker. An AA Degree in a related field and four years of experience in fields related including Community Health Work and client transportation or some combination of education and experience totaling six years. CPR Certification preferred.
Knowledge, Skills, and Abilities
  • Ability to empathize with and advocate for low-income households, and to maintain confidentiality with personal information
  • Must have the ability to drive in inclement weather
  • Must possess strong professional boundaries
  • Must have a strong sense of safety and adhere to safety protocols
  • Strong communication, interpersonal and customer service skills with a diverse population
  • Computer proficiency with MS Office required, with aptitude and ability to learn related technical skills
  • Ability to follow directions and work effectively in an open, fast-paced, high-stress setting with frequent interruptions
  • Good planning, organization and critical thinking skills, including ability to prioritize and communicate questions and concerns
  • Ability to work independently, using initiative and ethical judgment to resolve problems and ensure workflow is appropriately addressed in a timely manner
Training and Certifications

                Securement device training within 30 days of hire

PASS training within 30 days

CPR/First Aid Certification within 60 days of hire

Trained Community Health Worker within six months of hire        

Work Environment/Physical Activities:

Work is conducted in vehicles and client homes Work requires ability to walk clients to and from vehicle. Some lifting may be required.  May deal with difficult people, sometimes in crisis situations. This position requires local travel within Spokane County. Local travel will be approximately 50% of the position duties.
