Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) who's services include providing personalized, affordable, high-quality medical, dental, vision and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their individual needs. Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. East Valley serves the health care needs of uninsured and underserved individuals and families throughout our 8 health center locations.
Our mission is to provide access to excellent health care while engaging and empowering our patients, employees, and partners to improve their well-being and the health of our communities.
MAJOR POSITION RESPONSIBILITIES AND FUNCTIONS:

  • Maintains filing system of medical charts.
  • Assists with pulling out medical records during clinic hours.
  • Scans documents into electronic health record.
  • Routes to follow-up personnel all charts needing follow-up as indicated by providers.
  • Transfers records to the requesting agencies in a timely manner.
  • Logs transfer of records in patient's electronic health record in a timely manner.
  • Process and complete all subpoena requests in a timely manner.
  • Sort all incoming faxes and distributes to appropriate personnel.
  • Tasks provider patient requests incoming via phone message or fax.
  • Purge and send charts to storage.
  • Report to all scheduled shifts in a timely and consistent manner.
  • Other tasks as assigned by supervisor.
POSITION REQUIREMENTS AND QUALIFICATIONS:
  • Excellent customer service skills including good phone manners to care for patients and staff on a continual basis.
  • Must have basic computer skills to include, keyboarding and basic Windows and accurate data entry.
  • Must be organized, detail-oriented and works with a sense of urgency.
  • Exceptional patient relations and interpersonal skills required.
  • Knowledge of referral procedures preferred.
  • Knowledge and experience with EHR, Nextgen preferred
  • Bilingual English/Spanish (Highly preferred)
  • Excellent interpersonal skills

DOE: $22.00 - $25.98
