POSITION SUMMARY:

The Community Health Worker (CHW) is a trusted connection between the community and local health and social service systems. In this role, youโ€™ll build meaningful relationships, help individuals navigate care, and empower community members to improve their health and well-being.

This position plays a key role in reducing health disparities by providing outreach, education, advocacy, and support. The CHW works closely with healthcare providers, public health programs, and community organizations to ensure individuals have access to the resources they need.


ESSENTIAL JOB FUNCTIONS:
  • Serve as a liaison between community members and healthcare, public health, behavioral health, and social service systems

  • Conduct outreach and community engagement activities to promote health and wellness

  • Help individuals access services and resources, reducing barriers to care

  • Provide informal counseling, social support, and health education

  • Advocate for individuals and communities to improve access and quality of care

  • Identify community health needs and assist in developing strategies and resources to address them

  • Support and participate in community meetings, coalitions, workshops, and events

  • Collaborate with Project Coordinators and ECDHD programs

  • Maintain accurate documentation and follow program requirements

  • Participate in trainings and professional development opportunities

  • Perform other duties as assigned


REQUIRED SKILLS:
  • High school diploma or equivalent

  • Willingness to obtain a Community Health Worker certification within one year of hire

  • Strong communication, organization, and interpersonal skills

  • Ability to work effectively with individuals from diverse cultural, socioeconomic, and educational backgrounds

  • Basic computer skills, including Microsoft Office

PREFERRED SKILLS:

  • Bilingual in Spanish or Somali

  • Experience working in community outreach, healthcare, or social services


WORK ENVIRONMENT & PHYSICAL REQUIREMENTS:
  • Primarily office and community-based work
  • May require extended periods of sitting, as well as occasional standing, bending, and lifting (up to 35 lbs.)
  • Use of standard office equipment such as computers, phones, and copiers
