Job description

Position Title: Case Manager

Department: Programs

Reports To: Director/Manager of Case Management

Direct Reports: Volunteers, Interns

Status: Full Time/Non-Exempt


Summary:

The Case Manager at KARM works directly with guests facing challenges such as housing instability, unemployment, substance use recovery, trauma, and mental health barriers. Through ongoing assessment, goal-setting, and life planning, the Case Manager helps guests identify and overcome obstacles while connecting them with internal and community resources. Daily case load responsibilities include referral coordination, advocacy, documentation, and collaboration with staff, volunteers, healthcare providers, employers, and the housing department.


The Case Manager also helps maintain a safe, Christ-centered environment that fosters accountability, dignity, and life transformation, while supporting volunteer engagement and guest mentoring within professional and ethical boundaries. This role requires compassion, consistency, sound judgment, and strong organizational skills, with a genuine commitment to walking alongside guests on their path toward long-term stability and growth.


Duties:

  • Maintain an active residential program caseload by conducting assessments, developing individualized service plans, monitoring progress, providing support and crisis intervention, and encouraging accountability, recovery, life skills, and personal growth within a Christ-centered environment.
  • Coordinate internal and external referrals and supportive services including healthcare, behavioral health, recovery programs, housing department, employment, education, transportation, legal aid, and public benefits while assisting guests with applications, appointments, transition planning, and aftercare support.
  • Maintain accurate and timely documentation including case notes, assessments, referrals, incident reports, and HMIS/Mission Tracker records while ensuring confidentiality, compliance with policies and program standards, and adherence to safety guidelines.
  • Collaborate with ministry staff, community partners, churches, employers, treatment providers, volunteers, and interns to support effective guest care, continuity of services, and a safe, respectful, recovery-oriented environment consistent with KARMโ€™s mission and values.
  • Participate in staff meetings, interdisciplinary discussions, trainings, ministry initiatives, outreach efforts, and other assigned duties supporting program operations and guest success.
  • Ensure consistent application of program standards, curriculum, discipleship models, safety guidelines, and ministry policies while supporting guest engagement and growth in KARM programs and services.
  • Develop Christ-centered relationships that model a healthy Christian lifestyle and foster encouragement, accountability, compassion, and hope among guests, residents, staff, volunteers, and donors.
  • Perform other essential duties as assigned.


Requirements:

  • Bachelorโ€™s degree in Social Work, Psychology, Counseling, Ministry, or related field preferred; equivalent education and experience considered. Masterโ€™s level education and/or LMSW preferred.
  • Minimum of two years of experience working with individuals experiencing housing instability, substance use recovery challenges, mental health barriers, or related life challenges.
  • Ability to clearly communicate and model the Christian faith, including explaining the plan of salvation, while demonstrating compassion, trust, consistency, encouragement, and hope.
  • Demonstrated ability to work cooperatively with staff, leadership, volunteers, and community partners in support of ministry goals and participant care.
  • Strong interpersonal, coaching, counseling, goal-setting, organizational, administrative, and time-management skills, with the ability to maintain accurate records, adapt to change, manage competing priorities, and maintain healthy professional boundaries.
