$23.90 - $31.10/HourlyActual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.
Overview
Recruits patients into grant programs: answers incoming inquiries, assessing eligibility, enrolling patients, and scheduling appointments. Oversees daily operations and workflow for the department's grants. Assesses social, functional, and financial patient needs as it relates to their ability to access healthcare, and connects patients with appropriate cancer screening and prevention services. Collaborates with the referring provider/practice, diagnostic center scheduling, free clinics, etc to ensure all guidelines and requirements are met for appropriate patient care, follow-up and billing/financial assistance. Manages all aspects of data entry to fulfill grant requirements. Attends health fairs, screenings, and meetings to provide information about grant programs and Riverside Health Cancer Services related to breast health, women's health issues, prostate cancer screenings, HPV vaccination, smoking cessation, lung cancer prevention and early detection, and colon cancer screening. Participates in collaborative events/offerings occurring with Cancer Services and their community partners, such as the American Cancer Society Relay for Life events, etc. Attends annual Outreach Worker retreat as offered by VDH state office. Oversees volunteers and related programs and activities.
What you will do

  • Performs daily grant activity duties i.e. recruit patients, answer incoming inquiries, assess eligibility, enroll patients, schedule appointments, clerical duties and record keeping/data entry.
  • Provides timely follow-up regarding requests and concerns. Documents in the Electronic Medical Record, handling follow-up notes from physicians. Works closely with the supervisor to resolve needs of the patient according to physician notations.
  • Participates in health fairs and education programs that are offered to our local community. Oversees and works with the Riverside Health volunteers and Bonner Scholars from CNU to develop programs for support. Responsible for developing materials for the events. Develop and lead survivorship events, working with supervisor and volunteer program.
  • Assess, address, and assist with social, functional, and financial patient needs as it relates to their ability to access healthcare screening and prevention services. Assist with coordination of patient's transportation needs, food insecurities, lodging needs, etc, as needed.
  • Responsible for keeping records and reports required by the supervisor for the grant programs. Account for expenses and secure reimbursement through the ACS grant and Riverside Health Foundation, as needed/required.
  • Responsible for setting up the room and handling calls for various programs, Peninsula & Middle Peninsula. May assist the patient in referrals to ACS resources such as wig fittings under the direction of the ACS volunteer, Patient Navigator or supervisor.
Qualifications
Education
  • High School Diploma or GED, (Required)
  • Program Graduate, AMA approved radiologic technology program (Required) or
  • Bachelors Degree, Nursing/Sociology/Social Work/Public Health (Required)
Experience
  • 2 years healthcare experience, including electronic medical record experience (Required)
Skills and Abilities
  • Bilingual, Spanish Speaking Preferred

Hours: Primarily days, but some nights and weekends are required occasionally.
