As a Licensed Clinical Social Worker, you will be tasked with a variety of essential duties:

• Conduct thorough psychosocial assessments of patients to evaluate their needs, identify strengths, and address challenges hindering their mental health progress.
• Develop and deliver targeted counseling sessions focused on distress management, health education, and adaptive coping mechanisms tailored to individual patient circumstances.
• Collaborate with clients and an interdisciplinary healthcare team to devise personalized care plans that aim to positively influence behavior modification, encourage resilience, and promote sustainable lifestyle changes.
• Engage patients in goal-setting exercises during synchronous and asynchronous sessions, equipping them with the strategies needed to manage chronic conditions and their associated psychological concerns.
• Implement regular outreach initiatives through innovative digital communications methods, ensuring that patients feel supported and motivated to engage in their treatment journey.
• Diligently maintain comprehensive documentation of patient interactions, treatment plans, and progress updates within our electronic health record (EHR) system, ensuring compliance with established protocols.

You will be working with a diverse client base within our community health clinic, including individuals from various cultural backgrounds, age groups, and socioeconomic statuses. Our client population presents a range of mental health challenges, including anxiety, depression, trauma, and chronic stress. To be considered for this role, candidates must possess the following:

• A Master’s degree in Social Work from an accredited institution.

• A strong understanding of evidence-based therapeutic modalities, with the ability to apply them effectively in a clinical environment.

• Proficiency in utilizing electronic health record (EHR) systems for record-keeping and patient management.
