Back On Track Orthopedic Center is seeking a dedicated Full-Time Physical Therapist who will play a crucial role in enhancing the recovery and rehabilitation journey of patients in our outpatient and home care settings. Work Environment

This full-time role is primarily onsite, engaging directly with patients in both a clinical space and through home care visits. Key Responsibilities
  • Examine, evaluate, and assess patients with orthopedic conditions to determine the best treatment pathways.
  • Develop individualized treatment plans focused on achieving specific functional goals, while considering each patientโ€™s unique background and lifestyle.
  • Implement therapeutic interventions using evidence-based practices, and continuously monitor and adjust treatment plans based on patient progress.
  • Foster open communication with patients and their families, providing education on treatment protocols, care strategies, and activity modifications to enhance recovery.
  • Document all patient assessments, progress notes, and treatment plans accurately and timely in accordance with regulatory standards and best practices.
  • Collaborate with interdisciplinary teams, including physicians and nursing staff, ensuring seamless patient care coordination throughout the continuum.
  • Supervise and mentor support staff and students, promoting professional development and knowledge transfer within the team.
  • Engage in community outreach initiatives to promote the benefits of physical therapy and wellness, reinforcing our commitment to health education.
  • Participate in continuing education and training, staying current with the latest advancements in physical therapy practices.
Patient Population / Client Focus

As a Physical Therapist at Back On Track Orthopedic Center, you will primarily serve a diverse patient population dealing with a wide range of orthopedic issues, including sports injuries, post-operative rehabilitation, and chronic pain management. Qualifications
  • A Doctorate in Physical Therapy from an accredited institution is required.
