Job description

  • Represents Alumni Services Department for all 11 agencies
  • Attends Agency territorial & departmental meetings in their territory
  • Updates their territory on Alumni announcements
  • Communicates and collaborates with agency leadership and care teams to ensure clients have a smooth transition into the Alumni Community upon discharge
  • Analyzes client tracking reports in their territory to determine alumni eligibility
  • Verifies discharged client information in EMR is updated in the post-discharge tracking & engagement SalesForce dashboard
  • Sets up client introduction to the Alumni Community when requested
  • Explains Alumni Community to clients and ensures clients comprehend Alumni Community eligibility and guidelines
  • Completes post-discharge follow-up calls and information emails regarding the Alumni Community
  • Partners with admissions when former clients/current alumni members request help finding treatment or returning to the program
  • Partners with communications department to help find testimonials from former clients
  • Facilitates alumni community events and meetings
  • Collaborates with CaredFor team to resolve any app issues or questions
  • Ally with Business Development team when any former client request help finding an outside provider (therapist, etc)

Required Qualifications:
  • At least one year minimum of working with patients in the Substance Use Disorder (SUD) field
  • Associate's Degree or higher preferred
  • This role is remote, but candidate must reside in one of the following states: CT, FL, GA, IN, KY, ME, MA, NH, OH, RI, VA, NJ, NY, PA, TX
  • 2 years of lived experience in continuous recovery from any Substance Use Disorder (SUD) and/or personal experience with addiction in family members or loved ones preferred
  • Strong working knowledge of Microsoft Applications (Outlook, Teams, Excel, Word, etc.) and a CRM (Specifically SalesForce), and EMR software
  • Must be willing to travel
  • Pay rate is $26-$31 hourly, contingent on education, experience, and any applicable professional licensure
