Job description

Aftercare Coordinator

The Aftercare Coordinator works with graduates and program participants post attendance from the Camp Hope Program. These services include aftercare planning and alumni services for veterans and their families. This position is responsible for coordinating an assigned caseload of combat veterans diagnosed with PTSD that have graduated or attended the Camp Hope Program.

Key Essential Functions:

  • Foster the relationship of veterans and their families who have attended the Camp Hope Program and may need more intense services to tap into resources to adjust to the community.
  • Attend appointments with clients who need additional support.
  • Coordinate contact relationship for aftercare clients and assists with clients continuing care plan.
  • Engage and enroll struggling veterans, while meeting the unique needs of our aftercare clients.
  • Identify and establish relationships with community support networks.
  • Provide needed services through referrals and follow-up on progress.
  • Maintain a professional relationship with other service providers.
  • Knowledge of PTSD, general mental disorders, psychosocial aspects of PTSD, and understanding of the needs of former residents and alumni.
  • Coordinates with local authorities to conduct health and wellness checks.
  • Complete additional tasks/reports as requested by the supervisor and meets productivity standards as outlined in operational guidelines.
  • Work with Camp Hope program staff on establishing an aftercare plan for residents in the last phase of their program.
  • Monitors client progress with treatment goals.
