Do you love helping children and families thrive?  TFI Family Services is hiring a full-time Aftercare Case Manager to join our team!  

What You'll Need
  • A bachelor's degree in human services.
  • A valid driver's license.
  • A willingness to learn-we'll provide all the training you need!
A Day in the Life

As an Aftercare Case Manager, you'll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You'll:

  • Focus on risk management and help families maintain stability during the aftercare period.
  • Provide crisis intervention and work with public and private agencies offering supportive services.
  • Maintain accurate documentation and reports.
  • Coordinate community and educational services for families.
  • Meet with families in-person to support reunification and resolve any concerns.
