CLINICAL CASE MANAGER ASSISTANT - (LPN)

Job Summary:
Clinical Case Manager Assistant will be responsible for assisting the Clinical Case Manager in reviewing intake paperwork to distinguish acceptance, denial or request additional information from hospitals. Reviewing verbal orders for new admissions in Skilled Nursing Visits, Therapy, and Private Duty Nursing. Signing off on and reviewing therapy medication reconciliations. Other ad hoc projects will be handed off with time and experience.

Job Responsibilities:
-Review all PAR packets (initial, renewed, denied, pended, for all LOB's)
-Review all Skilled Nursing Visits intake paperwork to either accept, deny, or request additional information from
hospital
-Receive verbal orders for new admission for Skilled Nursing Visits
-Receive verbal orders for Therapy if needed
-Take/Receive recert verbal orders of PDN/C.N.A/Skilled Nursing Visits (new workflow CCM's are doing in Denver
now)
-Review all therapy medication reconciliation and sign off on
-Create pre-auth 485's for PDN or CNA
-Help CM's gather educational information for their patients and upload to parent portal (Part of COP's)
-Track Missed Visits

Job Requirements:
- Licensed Practical nurse with Degree from an accredited institution
- CPR Certified
- Preferred experience with administrative paperwork
- Basic Computer Competency (Word, Excel, EHR)
