MAJOR RESPONSIBILITIES:


The Hud Rapid Rehousing Program Senior Case Manager reports to the Program Director and is responsible for assisting consumers who are at-risk of homelessness and who are literally homeless. He/she is responsible for providing troubleshooting and assistance to case managers in the delivery of services to households. The Senior Case Manager is responsible for coordinating services to program participants and supporting the case management team.

DETAILED RESPONSIBILITIES:
  • Assist families to remain housed or to obtain permanent housing and support the household with on-going services to ensure housing stability
  • Supports and mentors case managers by providing leadership and troubleshooting issues
  • Provide advocacy for public benefits to include, but not limited to, Public Assistance, Social Security, etc.
  • Helps to train and supervise new staff according to program guidelines
  • Assist with household budget counseling and other individualized support to families
  • Assist in developing Policy and Procedures
  • Prepare housing stability plans with a path to permanent housing and self-sufficiency
  • Help capture, organize, and maintain program information, including data entry of participant information and monthly activity.
  • Attend all team meetings and provide regular updates of housing-based activities
  • Ensures that all housing and programmatic activities meets the mandates as set forth in the contract
  • Ensures that all housing and programmatic activities meet the mandates as set forth in the contract.
  • Assume other essential and non-essential functions to maintain work flow of program

REQUIRED QUALIFICATIONS:
  • Excellent verbal and written communication skills
  • Possess in-depth knowledge of community programs and services
  • Strong organizational skills and meticulous attention to detail
  • Team Player with good listening skills
  • Proficient in the use of Word, Excel and Power Point
  • A car for visits to Consumers

  • REQUIRED Valid New York State Driver's License and own insured Vehicle with 100/300 bodily injury liability
  • REQUIRED PPD TEST WITH PROOF
