Family Stabilization Case Manager

Hours: Monday - Thursday

Status: Full Time

Reports to: FHS Supervisor/ Manager

Purpose:

Under the direct supervision of the FHS Supervisor, this position provides direct case management for Anthem members for services in homeless prevention, applications for rental and/or utility assistance, Rapid re-housing due to financial hardship.

Duties, Functions and Responsibilities:
  1. Conduct initial interview/intake and client needs assessments based on referrals received.
  2. Determine eligibility for homeless prevention, rent and utility assistance and RRH based on criteria set in grant.
  3. Collect appropriate documents from client(s) to properly document eligibility (Identification of all household members, income, crisis/need, lease, sustainability, MCO designation, etc.)
  4. Must be aware of community resources so that correct referrals and information is given to clients to best meet their individual needs.
  5. Provide case management and supportive services to clients participating in housing program.
  6. Create individualized case plans with each adult member of the household, based on clients' needs, with the goal of achieving self-sufficiency.
  7. Review and update client's case plan monthly, and track outcomes.
  8. Connect client with wrap-around services through other programs offered at HELP, including WORC, Diaper Bank, holiday programs, etc.
  9. Connect client with mainstream programs, including but not limited to, Medicare, WIC, TANF, SNAP SSI/SSDI.
  10. Provide client with referrals and resources for other social services and partnering agencies.
  11. Enter case notes in true time data (daily) into HMIS, reflecting activities of the client's progress.
  12. Maintain current and accurate client files.
  13. Maintain clear boundaries with all clients and providers.
  14. Must never allow yours or the organization's ethical standards to be compromised.
  15. Other duties, as may be assigned.


Knowledge, Skills and Abilities:
  1. Knowledge of local social service agencies and mainstream programs.
  2. Knowledge of social service work, interviewing/referral techniques.
  3. Experience and understanding of homeless issues.
  4. Understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
  5. Must communicate effectively with persons from a variety of social, cultural and economic backgrounds, and possess a demonstrated understanding and sensitivity to culturally diverse populations.
  6. Effective written, verbal and listening skills. Independently initiates and responds to correspondence.
  7. Ability to maintain order and exercise appropriate judgment in crisis situations.
  8. Must be flexible and have the ability to quickly adjust to shifting priorities.
  9. Establish and maintain cooperative working relationships with consumers, fellow employees, other agencies and the general public.
  10. Organize work schedule and effectively manage time.
  11. Understand and follow all agency policies and procedures.
  12. General computer knowledge.
  13. Ability to work flexible hours, as required by consumer case load.
  14. Ability to perform accurate mathematical equations to calculate client's rent and utility costs, and to request funding to pay those bills.


Qualifications, Education and/or Experience:
  1. Bachelor's degree in Social Work or related field, or combination of education and experience, substantially equivalent to a bachelor's degree.
  2. Experience working with homeless individuals, families and children.
  3. Must pass a background check and pre-employment drug screening.
  4. Must have a current NV driver's license and clear DMV record. Driver History Reports are required to be submitted on a quarterly basis.
  5. Bilingual preferred.


Working Conditions:

Work is performed in an office setting, and at various community and residential locations. May be subject to sitting, standing, walking, bending, squatting, lifting, pushing and pulling, and simple grasping.
