CASE MANAGER IExempt Non-Exempt
Responsible to: Program Manager
Unit/Department: SAMHSA II

Probationary Period: 3 months

PURPOSE OF POSITION

The Case Manager will conduct intake interviews, care plan development, care coordination, referral services, substance use counseling coordination, health education, and social services navigation to members with substance use and other chronic health conditions. Other interventions are Healthy Conversations, RESPECT & Seeking Safety. The Case Manager will act as an intermediary between providers and members serving in numerous roles such as educator, evaluator, service coordinator, community resource researcher, and more.

RESPONSIBILITIES

The major responsibility of this position includes, but are not limited to:
  • Completes initial assessment as needed and reassessments of all assigned clients in a timely manner in-order to develop individualized treatment goals for each client.
  • Establishes treatment goals and objectives with clients as part of the interdisciplinary team.
  • Provides case management services including but not limited to weekly and monthly progress summaries, quarterly assessments, quarterly care planning, counseling, outreach, case conferences, advocacy, referrals (on-site & off-site) and discharge planning.
  • Ensure timely submission of clinical documentations to care organizations for uninterrupted services.
  • Coordinate transitional care from inpatient service to other settings as per care plan.
  • Coordinate detox referrals and transportation services as needed.
  • Conduct prescreening and intake assessment interventions with eligible individuals as needed.
  • Able to educate participants on Harm Reduction intervention and services.
  • Conduct overdose prevention education and Narcan dispensation as needed. Work collaboratively with team members to provide outreach and engagement too hard to reach clients.
  • Maintain documentation and update care plans at specific time intervals or as needed in c and Health Home electronic medical records system.
  • Assists clients to navigate the health insurance system including the AIDS Drug Assistance Program (ADAP) and/or other insurance networks.
  • Able to meet assigned deadlines for progress note documentation and service coding according to program Scope of Services.
  • Understands and is sensitive to chronic medical conditions, including HIV, mental health and substance use, diabetes, asthma, hypertension, heart disease and hepatitis diseases.
  • Performs other duties as requested.
EDUCATION
  • High School Diploma or GED, and one year of applicable experience and/or equivalent experience.

QUALIFICATIONS
  • Minimum one (1) year of similar work experience, facilitating groups and performing Outreach/community engagement activities.
  • Previous experience delivering evidence-based interventions such as 3MV, RESPRCT & D-Up!
  • Previous work experience with high risk, Harm Reduction and HIV+ populations.
  • Outstanding writing and communication skills; including multi-media outlets.
Compensation & Benefits


  • The base pay range for this position is $40,000 - $45,000 per year. *
