Kalos Health is searching for an RN Care Manager to join the team full-time.
Key Responsibilities:
- Complete a comprehensive assessment and develop a care plan for short-term & long-term needs utilizing clinical expertise.
- Manages active cases based on case intensity and acuity.
- Interacts continuously with member, family, physician(s), other providers and the Kalos interdisciplinary team.
- Assesses the options for care including use of benefits and community resources to meet member’s needs reflective in the plan of care and coordinates resources.
- Acts as liaison and member advocate between the member/family, physician and facilities/agencies.
- Maintains accurate records of case management activities in the electronic health record using clinical guidelines.
Required Qualifications:
- Minimum of 1 year experience in MLTC required.
- Current NY State RN licensure.
- 5+ yrs clinical experience, preferably in home health, physician’s office or public health.
- While case load is focused in a particular area, you must have the ability to travel within the 6 counties of Monroe, Genesee, Erie, Niagara, Orleans & Chautauqua as needed.
Preferred Qualifications:
- CCM certification preferred
- 2 yrs current case management or managed care experience preferred.
- Experience and knowledge with electronic health record software preferred.
- Bilingual skills a plus but not required.
