RN Care Manager

Key Responsibilities:

• Complete a comprehensive assessment and develop a care plan for short-term & long-term needs utilizing clinical expertise.
• Manages active cases based on case intensity and acuity.
• Interacts continuously with member, family, physician(s), other providers and the Kalos interdisciplinary team.
• Assesses the options for care including use of benefits and community resources to meet member’s needs reflective in the plan of care and coordinates resources.
• Acts as liaison and member advocate between the member/family, physician and facilities/agencies.
• Maintains accurate records of case management activities in the electronic health record using clinical guidelines.

Required Qualifications:

• Minimum of 1 year experience in MLTC required.
• Current NY State RN licensure.
• 5+ yrs clinical experience, preferably in home health, physician’s office or public health.
• While case load is focused in a particular area, you must have the ability to travel within the 6 counties of Monroe, Genesee, Erie, Niagara, Orleans & Chautauqua as needed.

Preferred Qualifications:

• CCM certification preferred
• 2 yrs current case management or managed care experience preferred.
• Experience and knowledge with electronic health record software preferred.
• Bilingual skills a plus but not required.
