JOB TITLE: Licensed Practical Nurse

STATUS/TYPE: Non-Exempt

REPORTS TO: Program Director

JOB PURPOSE: Perform routine nursing responsibilities in caring for the Adult Medical Psychiatric clients the scope of nursing licensure and guidelines set by physicians, treatment team, and other clinical staff in accordance with EHCS BH and procedure, local, state, and federal standards/requirements.

ESSENTIAL JOB FUNCTIONS include, but are not limited to:

1. Accountable for drug storage, count and administration
30% Weight

PERFORMANCE STANDARDS:
Administer prescribed medications as ordered by the physician.
Maintain clinical competencies specific to Adult/Geriatric/Medical Psychiatric population.
Transcribe physician orders accurately.
Participate in weekly treatment team meetings and daily treatment planning meetings.
Assists client assistance program. (Free Medication)
Conduct educational groups daily as scheduled.

2. Provide basic nursing care within the scope of practice and in compliance with aforementioned policies, procedures and standards.
30% Weight

PERFORMANCE STANDARDS:
Conduct assessments, treatment plans, and document in patient chart in a timely manner.
Insure the lab work and x-rays are done as ordered and reported to supervisor.
Perform admissions, discharges and transfers to clients according to policies and procedures of the facility.
Perform assessments of clients medical needs and reports this information to the charge nurse/physician in a timely manner.
Perform client care as directed and document accurately and concisely on clients behavior and participate in treatment.
Document behavioral changes and report these to the charge nurse/physician in a timely manner.
Participates as an active member of treatment in planning therapeutic approaches for individual patients.
Attends treatment planning and progress review meetings.
Provide quality client care by complying with physicians orders, clinician directives, and treatment plans and communicating with resources when needed.
Assistance with transportation for clients as needed.
Audit chart information for compliance with standards and report discrepancies.
Conduct education groups daily as scheduled.
Documents groups as per needed in a productive timely manner.

3. Maintain an appropriate knowledge of Psychiatric theories, chemical dependency and practices and applies this knowledge on patient care and treatment where indicated.
20% Weight

PERFORMANCE STANDARDS:
Attend all in service meetings and complete the required annual test to remain in compliance with state and federal educational requirements (ie: body mechanics, infection control).
Demonstrate the ability to carry out responsibilities for fire drills, codes, orientation of new staff and other areas as directed.
Keep the following certifications current: CPR, CPI and nurse licensure.

4. Performs other duties and special projects as required.
20% Weight

PERFORMANCE STANDARDS:
Respond to all inquiry calls and present information to physician and Program Director.

GENERAL PERFORMANCE CRITERIA
Professional/Technical

MINIMUM QUALIFICATIONS
Two (2) years experience as an LPN, preferably in a Psychiatric setting.

SPECIAL SKILLS AND KNOWLEDGE REQUIREMENTS

Work is of medium demand. Prolonged, extensive or considerable standing and walking; Continuous talking; listening; Occasional lifting; pushing; pulling and transferring of patients; Frequent reaching, stooping, bending, kneeling, crouching actions; Near visual acuity and ability to distinguish color tone and variation; Frequent, repetitive digital and hand actions.
