Case Manager/LVN
We are seeking an experienced healthcare professional to join our skilled team as a Case Manager (CM) to be responsible for providing skilled client’s case management. The CM is a dedicated individual who shares our common values such as serving with a passion and doing the right thing. The CM will provide excellent customer service to patients, physicians, pharmacies, facilities, nurses, and ensures the charts, intake, and scheduling is completed professionally and accurately.
Essential Duties and Responsibilities
- Case management: Conduct intakes, schedule nurses, patient contact, account contact
- Weekly skilled billing | Mobile Shift Management
- Quality Assurance
- Client and patient discharges
- Skilled medical staffing
- Facilitate nurse performance reviews
- Principles, practices and professional standards of medical records management, practice procedures and terminology
- Interpret and apply policies and regulations
- Exercise good judgement and tact when relating to health care providers and beneficiaries
- Communicate effectively with clients, patients, pharmacies, nurses, and team members
- Work together with Director of Nursing, Home Health Coordinator, ADON, and Clinical Supervisor
- Availability to be on-call for texts, calls, and Notifyd messages on a team rotation
Qualifications
- Active California LVN license
- Pass a background check and drug screen
- Certified CPR
- Negative TB skin test or chest x-ray
- Physical – Six months prior or 2-weeks after hire
- Adhere to HIPAA and maintain client confidentiality
- Knowledge of Medical Terminology
- Problem solving skills
- Experience with scheduling and billing, preferred
- Advanced degree, two years of experience in a patient care setting, and one year of case management experience, or any combination.
- Licensed driver with automobile that is insured in accordance with the organization’s requirements
- Self-directing with the ability to work with little direct supervision
- Ability to take after-hours on-call on a rotating schedule (respond to texts/calls/Notifyd messages)
- Treat clients, team members and the public with courtesy, respect and presents a positive public image
- Always respond to and maintain confidential information
- Able to prioritize and complete work on-time, and be detail-oriented

Pay rate: $1,600 week salary exempt
