Role Overview
Join our team at Hale Kipa as a full-time Transition Specialist that will provide Cognitive Restructuring groups for the youth, to assist them with learning positive ways to manage their thoughts and ultimately their behaviors so that when they are back out in the community, they will be able to function appropriately and be contributing members of society. The Transition Specialist will also participate in HYCF treatment team meetings, assist with transition planning pre-release, and will be responsible to manage assigned cases to ensure that coordinated services plan goals & objectives are being met post-release. Other responsibilities are to support the youth's transition back into the community and assist youth and their families in accessing community resources and continuing education or obtaining employment.
Qualifications
- Bachelor's Degree in Social Work, Psychology, Human Services, Counseling, or related field
- One (1) year of experience working with youth ages 5-19 in obtaining basic skills, goal setting, healthy living, relationships, and other aspects of prevention services.
Requirements
- Exceptional verbal and written communication skills.
- Proficiency in navigating electronic health record systems.
- Consistent and timely documentation.
- A valid driver's license.
- Current auto insurance & safety check required.
- Access to a vehicle for the purpose of transporting young adults.

Salary
- $40,000 salary
- Retention Bonus: $500 after 1 year of employment
