Job Title:             STAR Community Services Bilingual Case Manager

Job Summary:

Servicios de La Raza (SDLR) mission is to provide responsive and effective services to historically poorly served and vulnerable populations of Denver City and County. The Support Team Assisted Response (STAR) Community Services Program will target the following population: community members who are experiencing problems related to mental health, depression, poverty, homelessness and/or substance abuse issues. The Community Services Program will be a partner program of STAR, which is a program that sends a Paramedic/EMT and a Behavioral Health Clinician to low-risk behavioral health calls to de-escalate and connect a resident in distress with appropriate services.

STAR Community Services Program Case Manager will provide culturally and linguistically appropriate trauma-informed services, including behavioral health screenings, assessment, linkage to care and other wrap-around services, as needed. STAR services are deployed in response to low risk calls about persons not at imminent risk. Servicios’ model of service is grounded in principles of trauma-informed care and harm-reduction practice to provide culturally and linguistically responsive care to persons in moments of crisis.

Duties and Responsibilities: Position Specific

  • Provide culturally and linguistically trauma-informed case management services to clients
  • Provide client-centered and harm reduction approaches
  • Complete all appropriate assessments with clients
  • Create and update safety plan(s) with clients
  • Render in appropriate crisis intervention(s) with clients
  • Engage in community outreach and program education
  • Collaborate with STAR Program community partners
  • Provide appropriate internal and external referrals, as needed
  • Provide support to mental health therapists, as needed
  • Take an active role as liaison between outside agencies and clients
  • Complete and maintain timely and detailed records of all required client documentation and paperwork, including client interactions, which conforms to standards of best practice, and SDLR Policies and Procedure
  • Maintain confidentiality of client records
  • Attend networking meetings and connect with businesses, service agencies and community organizations to form partnerships to provide resources for clients
  • Attend all staff trainings, in-service trainings, conferences, and development meetings as assigned by supervisor
  • Report tracking and development to the Program Director
  • Attend and participate in weekly meetings, supervisory sessions, and special events


Duties and Responsibilities: General


  • All members of the SDLR workforce are expected to support the agency’s community outreach, engagement, and recruitment efforts.
  • All members of the SDLR workforce are expected to support and engage in advocacy for social justice, community outreach and engagement, and bringing voice to the voiceless.
  • All members of the SDLR workforce are expected to support the agency’s fundraising objectives. Tasks might include activities like active fundraising, grant development, solicitation of donations, agency ambassadorship, and informational outreach, and public speaking.
  • Maintain a respectful and collaborative working relationship with all members of Servicios’ workforce
  • Participate in all required internal and external meetings
  • Other duties as assigned.

Required Skills:

  • Fluid competence in Spanish is required
  • Proficient bilingual Spanish/English oral and written language skills required
  • Utilize life experience(s) when appropriate
  • Commitment to and experience with equity, cultural relevance, diversity, social justice, and disparity reduction practices with persons of color, agencies of color, and un- and underserved communities
  • Ability to establish and maintain effective working relationships with clients, co-workers, agency partners, and the public
  • Cultural relevance and/or sensitivity to populations served
  • Computer literate, strong organizational skills, detail-oriented
  • Ability to be flexible with work hours to include evenings and weekends
  • Minimum of two years’ experience with providing community-based care, especially in the delivery of care and services around urgent and emergent needs expressed by members of the community
  • Minimum of one to two years with crisis management, de-escalation and conflict resolution
  • Knowledgeable in trauma-informed care and experience in working with highly traumatized community members


  • Minimum Requirements:
  • Valid Drivers License and Reliable Transportation; in-person outreach is apart of the roles and responsibilites


  • Required Education and Credentialing:
  • Bachelor’s degree in psychology, human services, social work or other related field, preferred but not required
  • Relevant years of experience will be considered

Working Environment and Physical Requirements:

  • 80+ person workforce situated across multiple regional offices.
  • ADA accessible; Active Safety Committee.
  • Ability to perform basic office duties.
