Salary: $43,000-$48,000/annually

Reports to:        Case Management Supervisor  

Status:               Salaried, Full-time, Exempt                                                                               


Position Objective:


  • Bachelor’s degree required
  • Three to five years of experience providing services to homeless or low-income persons preferred (may include employment, educational or volunteer experiences, internships, and academic studies).
  • Ability to engage and establish positive rapport with a diverse group of individuals experiencing homelessness who are on the street or in other locations not intended for human habitation
  • Ability to work cooperatively and productively with outreach team members and staff of partner agencies and community organizations
  • Exceptional communication skills and a strong commitment to a team environment;
  • Knowledge of human behavior to provide assessment, intervention and support services
  • Working knowledge of chronic medical and mental health conditions, substance abuse behavior, and best practice treatment approaches
  • Detail-oriented with strong organizational skills
  • Proficient in all Microsoft and Google applications and ability to master use of an electronic database
  • MUST HAVE a valid Indiana operator’s license in good standing and proof of state-required auto insurance.  Utilization of personal vehicle for daily travel within the community & client sites is required. Approved mileage is paid at current IRS reimbursement rate.

 

Essential Functions:


Case Work: 

  • Engage with a diverse clientele to provide case management intervention that may include but is not limited to assessment of client strengths, resources and needs; crisis intervention; information and referral; advocacy and support.
  • Responsible for assessing and enrolling clients deemed eligible for housing through the CES Housing Support project operated by Horizon House.
  • Assist with client conflicts or other disturbances, intake and outreach efforts within the agency and community.

 

Coordination of Services: 

  • Work collaboratively with client to develop effective housing plans with goals and action steps as well as to modify the case plan as needed.
  • Collaborate with community providers and ensure clients have access to services that may include addiction recovery support and/or treatment, medical care, mental health care and other resources and services specific to client needs and goals.
  • Provide ongoing support and advocacy with the goal of ensuring housing sustainability.

 

Record Keeping: 

  • Maintain appropriate and timely documentation including but not limited to client identifying information, service records and case notes.
  • Documentation of encounters with clients as well as services performed on behalf of clients will be compliant with agency and Indianapolis Continuum of Care standards and protocols and will be entered into the HMIS adopted by the Continuum and utilized by Horizon House.

 

Legal and Ethical Practice: 

  • Take appropriate action based on agency protocol and in accordance with the law.
  • Observe policies related to client confidentiality with respect to reports of abuse or neglect or other reports to law enforcement entities.
  • Maintain ethical conduct/behavior with clients, co-workers, and colleagues.

 

Case Conferences and Staff Meetings:

  • Participate in multidisciplinary and/or community case conferencing designed to advance client progress toward goals.
  • Attend and participate in Housing Team meetings and other Horizon House staff meetings as assigned. Represent the agency at community meetings/events as assigned.

 

Education/Training: 

  • Attend required agency education and training activities, including those that are position-specific. Participate in community training initiatives as available/assigned. Assist in training new staff and community partners as appropriate. 
  • Provide peer assistance to other case management and housing team members.

 

Research & Development: 

  • Participate in research programs such as the Point in Time Count and other programs when indicated. Participate in program development activities including but not limited to those related to policy & procedure, advocacy, funding, and evaluation with the Housing Supervisor or as assigned.

The Intensive Case Manager will regularly communicate with Horizon House employees, outside vendors and other individuals as deemed necessary in order to fulfill the responsibilities of the position.
