Job Summary
The Advanced Practice Provider (APP) is responsible for the health outcomes of an assigned patient population in collaboration with the physician. Functions as a member of a multidisciplinary health care team to ensure high quality, cost-effective care to patients in their area of clinical expertise and within the scope of their licensure and credentialed privileges. Follows established standards, procedures and practices and gives specific patient management directions to nursing and other staff. Incumbents in this position may be required to perform all or a significant combination of duties depending on their specialty area. The APP reflects the mission, vision and values of the organization and adheres to the organization’s Code of Ethics, Corporate Compliance program and all relevant policies, procedures, guidelines, and other regulatory and accreditation standards. The APP demonstrates critical thinking skills, ability to provide and delegate activities to staff based on patient assessment and the requirements to deliver care in a safe efficient manner maximizing positive outcomes of the patient.
Essential Job Functions
- General
- Works with the Hospitalist Group to ensure electronic medical record documentation, history/physicals and holding notes are entered on all assigned patients within 24 hours.
- Will use electronic medical record system to the highest capability to ensure reporting quality measures are met (i.e. E prescribe, PDMP, Care Continuity documents etc.)
- Pre-Surgical/Acute Care Evaluations
- Provides comprehensive pre-surgical/acute care clinical evaluations.
- Coordinates outside consultations as indicated.
- i.e. Cardiac, Pulmonary, Anesthesia etc.
- Orders, interprets, and signs off on pertinent diagnostic exams.
- Collaborates findings with the primary care provider and/or refers patients back to their primary care provider for follow-up of identified areas to achieve surgical optimization.
- Documents history and physical in the patient's medical record.
- In-patient/Extended Recovery/Observation Care
- Develops and maintains professional rapport with physicians and surgeons.
- Contributes to the economic and fiscal well-being of the department by practicing efficient utilization of resources.
- Ensures Medication Reconciliations are completed for assigned patients.
- Performs assessments for all assigned patients as needed with the clinical team.
- Responds in person to evaluate changes in patient condition and coordinates care with the treatment team as needed.
- Addresses urgent medical issues as needed (i.e. abnormal lab values, change in clinical status, or transfer to a higher level of care to ensure excellent outcomes.)
- Works with the assigned teams to improve clinical care, and quality outcomes for patients through collaboration with clinical team and key staff by participating in strategic initiatives.
- Communicate with the clinical team, patients, and families about the plan of care.
- Completes and signs all medical records as required.
- Works in conjunction with the clinical team, case management, nurse navigator, and utilization review to facilitate the continuum of care post-discharge.
- Provides Hospitalist on-call coverage which includes week and weekend day and night time call coverage as well as holiday coverage as scheduled and needed.
- Quality and Improvement
- Develops and monitors quality improvement of programs.
- Contributes to Hospital-wide QAPI process.
- Contributes, collaborates, reviews, and/or updates policies and procedures as needed or requested.
- Utilization Review
- Collaborate with clinical and administrative teams to evaluate the appropriateness of admission and duration of stay.
- Education
- Provide education to nursing and clinical staff throughout the facility as needed or requested.

Minimum Requirements
Education: Completion of an accredited Physician Assistant or Nurse Practitioner program.
Experience: 2 to 3 years relevant experience and/or training is required, pervious experience in related care area preferred.
Certification/Registration/Licensure: Current certification as a Physician Assistant (PA) or Nurse Practitioner (NP) through their national certification bodies. Licensure as a Registered Nurse and Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Colorado. ACLS, BLS, PALS, and DEA certifications required.
Physical Requirements:
Position requires lifting, carrying, pushing, and/or pulling up to fifty (50) pounds while administering patient care; stooping, kneeling and/or crouching; standing and walking five (5) or more hours per day; reaching, grasping, and handling and/or feeling five (5) or more hours per day. Visual acuity to correctly distinguish skin tone variations, evidence of IV infiltration, infection and to read telemetry, blood pressure gauges, etc; audio acuity to correctly distinguish various body sounds, i.e. respirators, heart sounds, bowel sounds and to discern sounds made by various types of machinery or equipment which would indicate dangerous or improper operation.
Working Conditions/Environmental Exposures
Environmental hazards such as chemicals, solvents, needles, needle containers, sharps, blood, body fluids, various infectious/communicable diseases, infectious medical waste, radiation, and/or radioactive materials and/or radioactive waste, proximity to moving mechanical parts. Protective clothing/equipment may be necessary.
Salary Range: $50.48-$84.62
