Ryan White Non-Medical Case Manager

Job Summary: This position is primarily responsible for providing case management services and coordination of all services needed by Ryan White Eligible Daily Planet clients, to include medical, mental health, dental, vision, substance abuse, housing, benefits, and insurance acquisition, as well as other social service needs under the Ryan White consortium. Protects the dignity, privacy and confidentiality of patients and their families; and works collaboratively with other Daily Planet providers and staff to promote a team-based approach to patient care. Participates in all safety programs which may include assignment to an emergency response team.

Job Requirements: Bachelor’s degree in a field of science required, Master’s degree in Social Work preferred. One (1) to three (3) years of general clinical case management experience required. Knowledge of Ryan White Part B Service Standards. Knowledge in chemical dependency, mental health, housing, and homeless service systems required. Skills include excellent oral and written communications including telephone, organizational, and personal computer including word processing and Microsoft office. Must demonstrate skill and knowledge in case management, client assessment, engagement, and group exercises.
