Job Title: Direct Support Professional 2
Status: Hourly: Full Time, Part Time, & Per Diem
Department: Developmental Disabilities Services
Scope of Service: The Direct Support Professional is responsible for a variety of duties related to the overall care of individuals with Intellectual Disabilities, enabling each person to live a valued and meaningful life. Support and care shall be delivered in the most respectful and dignified manner. The DSP assists each individual to live in a home of their own, develop and support reciprocal relationships, and become part of their community. The DSP supports individuals to learn and enhance activities of daily living, fosters involvement in social and community activities and supports personal care needs as required. CNA's and HHA's are encouraged to apply. Performs other duties as assigned. EOE.
QUALIFICATIONS:
- Bachelor’s or Associate’s Degree preferred with minimum of one years’ experience, working in a related human services field. High School Diploma or equivalent considered with a minimum of three years’ experience working in the field.
- Valid Connecticut Driver’s license with clean driving record is required.
- Certification as a Direct Support Professional preferred
- Completion of certifications of CPR & First Aid, PMT, and all others required through DDS and CoC.
- Computer skills required to navigate agency electronic record, access email, and complete documentation.
- Available to work evenings, weekends, and overnights as required.

Job Title: Behaviorist
Status: Salary
Department: DDS
Scope of Service: : Works with individuals with mental health challenges, intellectual disabilities, and co-occurring disorders to develops, implement, and review Behavioral Support Plans (BSP). Assesses skills, strengths, communication needs, and behavioral challenges to create person-centered behavior programs assisting the individual to continuously grow and achieve a fulfilling life. Completes behavioral assessments, analysis, and completes scheduled reports of assigned individuals. Provides ongoing training and education to staff for the successful implementation of the behavior program. Reports to the Eastern Division Director. Performs other duties as assigned. EOE.
QUALIFICATIONS:
- Full Time licensed clinical social worker, professional counselor, or BCBA or;
- Part time contracted consultant considered
- Licensed with CT State DDS to complete Behavioral Plans preferred.
ESSENTIAL JOB FUNCTIONS:
Clinical Services
- Works with Eastern Division Director to ensure compliance and delivery of all applicable behavioral program goals and objectives.
- Provides hands on education, training assessment & decision-making skills needed to assist in managing the program, and any associated issues on a daily basis.
- Facilitates and monitors effective day-to-day delivery of services within assigned programs.
- Monitors effective engagement between staff and clients, volunteers, and other identified program supports.
- Ensures that services are person-centered and responsive to individual needs.
- Attends and participates in agency, team, and inter-disciplinary meetings.
- Works to assist in preparing for quality and chart audits, maintains benchmark guidelines as needed.
- Plans and facilitates staff and house/community meeting.
- Facilitates coordination with other agencies and/or community services ensuring appropriate client care.
- Monitors documentation of services and the collection of data for contracted/agency reporting.
- Responds to crisis situations and provides appropriate interventions, de-escalation, and debriefing.
- Provides and documents program trainings for performance evaluations consistent with agency policy.
- Provides new staff with behavioral orientation and ongoing professional development and training.
- Consistently works in a respectful, constructive, and cooperative manner with fellow staff and providers.
- Upholds all Health Insurance Portability and Accountability Act (HIPAA) regulations and ensures all necessary releases of information for outside communication are obtained.
- Models culturally competent behavior interacting with clients, professionals/providers, and fellow staff.
- Ensures accuracy of electronic records and case files in accordance with programming, contractual, licensing, and accreditation requirements.
- Adheres to the standards of the Council on the Accreditation of Rehabilitation Facilities (CARF).
- Participates in agency committee work when available or as permitted.
- Completes all trainings as required per program contract and agency policy.
- Attends agency sponsored in-services, outside trainings, conferences and meetings to facilitate continued professional growth and ensure the quality of case management practices.

Continuum established its roots in the Greater New Haven area in 1966 with the creation of The New Haven Halfway House, its first residential program for persons with mental illness.
From its founding, Continuum has sought to demonstrate that previously institutionalized adults can live meaningful lives in the community with the right support and services. Today, we are one of the largest community housing providers in Connecticut for persons with mental illnesses and substance abuse. Continuum operates 55 licensed group homes, supported and independent living programs, crisis and respite services, and residential case management programs. Our grass roots laid the foundation for the development of a comprehensive array of residential service options for persons diagnosed with mental illness as well as individuals with developmental disabilities.
Our programs are all based in residential settings (homes and apartments) so that people can rebuild their lives in the community rather than in institutions, hospitals, or jail. Continuum serves more than 2,000 individuals annually in various locations throughout Connecticut. Most of the individuals serviced by the agency live below the poverty level and many have experienced chronic homelessness or challenges with addictions. We provide them with the support they need to break the cycle of homelessness and institutionalization so that they can live as fully and independently in the community as possible.
