Salary: $33,280-$36,000 per year plus commission

About Veterans Care Coordination (VCC):

VCC is a leading provider of services that help veterans and their families age safely and comfortably in their own homes. We are dedicated to honoring their service and providing them with the resources they need to live independently and with dignity.

About the Role:

In this hybrid role, you will play a vital role in supporting our veteran and family clients by:

Building trusting relationships with veterans and their families, listening to their needs, and providing support and resources as needed.
Coordinating and assisting with a variety of in-home services, such transportation, personal care, and medical supplies.
Providing referrals to community resources and government benefits programs.
Maintaining accurate client records and ensuring all information is kept confidential.
Collaborating with the family and home care agency to ensure client receive holistic support.

Who We Are Looking For:

Minimum 1 year of experience in customer service, social services, or a related field.
Strong communication and interpersonal skills, with the ability to build rapport with people from diverse backgrounds.
Excellent organizational skills and the ability to manage multiple tasks simultaneously.
Familiarity with veteran resources and benefits is a plus.
Proficiency in Microsoft Office Suite or Google Workspace is required.
Strong work ethic, a positive attitude, and a commitment to providing exceptional service.
