Job Description

Pathway Caring for Children is looking to hire a full-time Licensed Foster Care Case Manager to provide support to children and teens in foster care. The caseload size will vary based on the level of care required by individual clients and the number of foster homes served. This will be a hybrid position.


Responsibilities of the role include:

  • Conducting casework sessions with each child regularly and documenting progress in case manager progress notes.
  • Assisting children in working towards established permanency plans, such as reunification, adoption, independent living, etc.
  • Collaborating with the team and clients to stabilize mental health symptoms and preserve foster care placements.
  • Coordinating all services provided to the child.
  • Ensuring that team meetings are held as appropriate.
  • Assisting clients in acquiring skills to improve functioning levels and reduce symptoms of mental health disorders.
  • Completing all required documentation and training within established time frames.
  • Providing Mental Health CPST services to foster children through individual and/or group CPST sessions.
  • Offering emotional support, advice, and technical assistance to staff/foster care givers, including designing and coordinating Behavior Management plans and other treatment/change programs for children.
  • Conducting family problem-solving meetings when appropriate.
  • Providing child-specific training to foster care givers through home visits.
  • Maintaining availability for crisis intervention during assigned after-hours pager duty.
  • Participating in various in-service training programs designed to enhance job performance.

Qualifications:

  • An appropriate State of Ohio license, such as LSW, LISW, LPC, LPCC, CCDC, Counselor Trainee, Psychology Assistant, or other license specified by all ODJFS rules is preferred. For applicants not currently licensed, but eligible for licensure, we will train while working towards licensure and cover the cost of the examination,
  • A Bachelor's Degree.
  • At least two years of experience working with children and/or teens with mental health disorders.
  • Experience and aptitude in working with children in crisis situations.
  • Knowledge of working with children in foster/adoptive placements.
  • Demonstrated ability to work well with various types of people.
  • Possession of a valid driver's license, a good driving record, and the ability to drive to foster homes and other community locations.
  • Automotive liability insurance and a safely maintained car.
  • Must meet background screening requirements as defined in OAC code 5101:2-5-09 and be an assessor complying with ODJFS 5101:2-48-06 if performing assessor duties.
  • Demonstrates sensitivity and consideration for the cultural differences among the children and families served by Pathway.
  • Demonstrates knowledge of working with birth families.
