Lead Case Manager - Senior Services

There is a full-time, benefit eligible position available for a new project with the Senior Services program at Catholic Social Services. Working alongside the Senior Services Director, the Lead Senior Services Case manager will help to facilitate the strategic planning process to develop a sustainable framework for case management services for older adults.  Due to grant restrictions, there is a possibility that this position could end on December 31, 2024.

This position provides case management services directly to community dwelling seniors and their caregivers. This includes but is not limited to program assessment, care planning, advocacy, and coordination of services to meet biopsychosocial needs.

Master’s degree in social work, gerontology, public health, or related human services field preferred, or minimum of bachelor’s degree and equivalent combination of education and experience.  Must be professional and have excellent verbal, written, and interpersonal communication skills. Requires at least one-year direct client service experience, preferably with older adults, as well as knowledge of and experience with nonprofit human services agencies and resources throughout Washtenaw County.    

Competitive pay; will be dependent upon relevant experience. Total compensation includes benefit and retirement plans with employer contribution, plus paid time off and a generous paid holiday schedule which includes Christmas Eve through New Year’s Day.
