Job Description
Definition:
The Case Manager I/II works within a multi-disciplinary health care team in the delivery of comprehensive services in a community health setting. The Case Manager assists the client and the health care team in the development and implementation of a health care plan tailored to the individual needs of the client.
Reports to: Registered Nurse Supervisor or Program Manager of Women’s Health
Qualifications:
1. High school diploma or General Educational Development (GED) certificate, and completion of a State approved program for Medical Assistants. Licensed Vocational Nurse preferred.
2. Completion of state approved training and certification (Calif. Business & Professional Code 2069, 2070; and Calif. Code of Regulations 1377, 1377.1) relating to the performance of injections, skin testing, and withdrawal of blood.
2. Case Manager I position one (1) year minimum experience with Maternal and Child Health, counseling, health education, client advocacy, and community networking preferred. For Case Manager II position three (3) year minimum experience in Maternal and Child Health, CPSP and case management experience, counseling, health education, client advocacy, and community networking.
3. Baseline knowledge of Family Planning services and issues required. Completion of the Basic Health Worker Certification within 12 months of hire date and completion of the CPSP training program within 6 months of hire date.
4. Willing to travel to provide outreach presentations. Must have a current CA Driver's License, an available vehicle, and valid liability insurance.
5. Ability to perform a variety of tasks once trained.
6. Willing and able to work a variable, flexible schedule of 40 hours/week, Monday thru Saturday and evenings.
7. Be able to communicate tactfully and empathetically, with cultural sensitivity; with staff and patients. Fluently bilingual Spanish + English required, with excellent oral and written communication skills demonstrated in both languages. Must pass the bilingual oral and written examination within ninety days of employment.
8. Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy.
