We are seeking a full-time Social Worker & Discharge Planner for our skilled nursing facility.

Duties include but are not limited to:

• Ensuring each resident receives necessary services
• Each resident makes a smooth adjustment to the Home
• Each resident achieves the highest level of emotional and psychological functioning and independence
• Works with residents and families in admission process to ensure a smooth transition in admission.

Qualifications/Requirements:

• Previous Social Worker experience
• Experience working directly with individuals
• Must have Healthcare setting experience
• Must have experience with Medicare & Medicaid
• Maintain confidentiality
• Team cooperation
• Reliability in attendance & punctuality
• Work independently
• Good judgement
• Positive working relationships with staff, residents, families and outside contacts
• Adapt to changes
• Communicate efectively
• Emotional control under stress
