Position: Community Case Manager

Reports To: Senior Case Manager

Summary of Position:

The Community Case Manager will case manage single adult residents at various sites in Northern Virginia. Will work with area agencies to refer and identify individuals for services and supportive housing through the County'ss Coordinated Entry process. Works with residents and other service providers to develop a plan of service to meet social, health, emotional and economic needs to prevent or end homelessness. Coordinates services using a wraparound approach. Provides individual and group life skills training, socialization skills, and enrichment activities, including money management, cooking, nutrition, etc.

Essential Responsibilities:

Some of the essential responsibilities of the Case Manager include, but are not limited to:

  1. Advocates for and actively assist residents in obtaining services (e.g., health, mental health, intellectual disability, alcohol and drug, housing referrals, financial assistance, home-based services, training, medical services, mentoring, and socialization);
  2. Addresses and if necessary diffuse crisis situations with residents. Assesses safety issues and assists, residents, to access emergency services as needed (e.g., mental health services, adult protective services, women’s shelter, homeless shelter);
  3. Is proactive in ensuring a safe environment by encouraging medication compliance and understanding and effectively navigating the crisis cycle. Provides counseling and crisis intervention to residents;
  4. Models caring, concerned, respectful attitude toward residents. Maintains adherence to resident guidelines and is familiar with and complies with policies and procedures of New Hope Housing, Inc;
  5. Coordinates with residents and service providers to provide an environment that fosters increased stability and the opportunity to move on to more independent housing in the future;
  6. Facilitates house meetings, workshops, and groups on a regular basis;
  7. Assists residents with the upkeep of apartments by meeting regularly with residents in their units. Through life skills training, teaches residents simple household chores, upkeep, and cleanliness of their homes;
  8. Transports residents in a personal vehicle, as needed. Is able to operate the agency 15 passenger vehicle, when available;
  9. Meets with residents regularly and as needed to provide feedback. Regularly assesses, supports, and facilitates resident progress. Maintains case notes in the Homeless Management Information System (HMIS);
  10. Maintains records in compliance with agency policy and compiles requisite statistics;
  11. Attends interdisciplinary meetings regarding resident progress. Prepares clinical reports for the interdisciplinary team;
  12. Develops and maintains a positive relationship with residents, Residential Services Specialist (RSS) staff, Facilities staff, and members of the Permanent Supportive Housing Team;
  13. Works flexible hours to accommodate residents’ needs. The schedule may include evenings, weekends, and overnights, as needed;
  14. Shares on-call responsibilities with the Permanent Supportive Housing (PSH) Case Management Team (CMT);
  15. Identifies and addresses volunteer needs for the program. Develops and maintains a positive relationship with volunteers and donors;
  16. Performs other duties as assigned.

Required Qualifications:

  1. BA in behavioral science and considerable experience working with adults with serious mental illness;
  2. Ability to communicate effectively both orally and in writing;
  3. Experience with chronically homeless adults and adults with substance abuse issues;
  4. Strong counseling and crisis intervention skills;
  5. Flexibility, creativity, and leadership skills;
  6. Able to make decisions independently and work autonomously;
  7. Ability to demonstrate a commitment to the specific agency mission values and philosophy;
  8. Comfortable traveling and spending up to 75% of the time in the community;
  9. The willingness to undergo a background check and possess a valid driver’s license with a good driving record to be approved by our insurance carrier.
