Job Description
The Clinical Case Manager (CCM) will be a highly organized, efficient, and innovative individual with the ability to evaluate and prioritize the need for, and level of, service coordination. Possessing a strong understanding of clinically complex individuals, will track expenses and negotiate services that are the most appropriate and cost–effective, while continually evaluating the quality of services delivered.

Essential Responsibilities:

The CCM will perform all duties in compliance with NH Regulations (He-M-503). It is the responsibility of the CCM to review and understand all rules and regulation promulgated by the New Hampshire Bureau of Developmental Disabilities. Generally responsibilities include:

· Schedule and review assessments of a client’s needs for service, and design a seamless delivery method based on pre-determined criteria and the results of the assessment process; provide risk management leadership as needed for director coverage.

· Develop Service Plans with stated goals, objectives and methods based on client evaluation and all assessment materials.

· Complete and consistently update all required documentation within requested time frames.

· Complete reviews and written submission of routine documentation such as behavior plans, Community of Practice updates or referrals, Risks Assessments and progress reports.

· When necessary represent or intervene on client’s behalf to communicate their needs, desires and preferences.

· Develop proactive crisis planning and monitor for efficacy, being mindful of cost and efficiency.

· Negotiate service contracts with responding vendors and acquire services that are quality and value-based; ensure that services negotiated are being provided in an appropriate manner.

· Formulate client budgetary activities; assist in decision-making process regarding vendor financial reimbursement issues.

· Foster use of community-based services for continuum of care during program transition and end-of-life planning.

· As necessary make appropriate referrals for client and family members.

· Provide clinical case management for acute and moderate-to-high risk clients including:

1. Coordinate and track risk documentation and vendor compliancy.

2. Monitor behaviors for risk to self and others; make changes to plan as needed.

3. Identify training needs for staff; work with provider to coordinate training as needed.

4. Attend and fully participate in all aspects of Local Risk Management Committee.

· Provide crisis intervention.

· Write Risk Management plans and annual updates.

· Present to a clinical review team the comprehensive dynamics of high-risk case for planning purposes.

· Participate in Agency-wide committees, teams, and work groups in areas of quality improvement, utilization management, inter-departmental problem solving, etc.

· Attend court hearings as appropriate.

· Work in cooperation with attorneys, file court applications as requested and paperwork as needed.

· Work with intake and transition teams to determine needs of incoming clients.

· Visits clients in jail or New Hampshire Hospital as needed; work with teams for development of community- based services

· Travel out of state for client visitation/oversight on rare occasions. Teamwork, ability to work from office or remotely and face-to-face interaction are essential job functions.Other duties and responsibilities as assigned by the supervisor.

All staff must adhere to the NH state law requiring mandated reporting of suspected abuse, neglect, or exploitation. Additionally, staff are required to maintain criminal, motor vehicle and Bureau of Elderly and Adult Services record checks that meet agency standards. Staff are also required to travel and transport clients when applicable. This includes having access to a reliable vehicle, a valid driver’s license and maintaining personal auto insurance of $100,000/300,000.




Requirements

Qualifications:

Education and Experience

· Bachelor’s degree in Human Services or related field and 4 - 5years of experience working in human services; Master’s degree in Human Services preferred

· Demonstrated knowledge in field of clinical counseling, psychology or social work

· Demonstrated knowledge of the developmental disabilities and behavioral systems

· Understanding of utilization management

· Proficiency in various software applications including Word, Excel, and Access
