Job Description

Are you an experienced RN Field Case Manager looking for a new opportunity with a prestigious healthcare company? Daily Responsibilities of a RN Field Case Manager:

Performs intermittent skilled nursing services
Communicates the patient’s progress with other discipline
Directs, supervises, and instructs non-professional home health aide staff in the provision of personal care to the patient

Requirements:

1+ years of experience within a Home Health role
Clear and Active RN License within the State of Texas
Oasis EMR experience highly preferred

Hours for this Position

Monday to Friday: 8:00am to 5:00pm
No weekends!
