Core Responsibilities:

Provide case management support to up to ten individuals, fostering their journey through coordinated treatment and care.

Facilitate access to psychiatric evaluations, medication management, and essential health services.

Enhance team effectiveness by assisting the Team Lead with organizational tasks, compliance tracking, and staff training initiatives.

Engage in our Quality Improvement Committee and offer vital after-hours support to ensure continuous care.

Ideal Candidate:

Holds a Bachelor’s degree in Psychology, Social Work, or a related field.

Brings a background in human services, with a preference for those with three years of specialized case management experience.

Demonstrates exceptional problem-solving and crisis management skills, along with a commitment to ethical practices.

Qualifications Needed:

CPR & First Aid Certification, TB Clearance.

Valid Hawaii driver’s license.
