BECOMING OUR WRAPAROUND CARE COORDINATION SUPERVISOR - CCCS

As a Wraparound Care Coordination Supervisor - CCCS, your pivotal role within the Care Coordination and Support program involves promptly assessing referral information, conducting IM+CANS assessments, and updating documentation as per HFS guidelines. You collaborate closely with youth and families to develop Crisis Prevention and Crisis Safety plans, establish Child and Family Teams, and facilitate regular meetings. Engaging in weekly home and community visits, you link families to appropriate service providers and coordinate with the youth's MCO Case Manager. Your culturally competent and trauma-informed approach ensures effective collaboration with treatment providers, schools, and other contacts to address the youth's holistic needs. Maintaining timely and professional case records, you uphold the standards of the Care Coordination and Support Organization while managing a caseload ratio of 1:10 within your Designated Service Area.

All that we need from you is the following:

• 21+ years of age
• Bachelor's Degree in counseling and guidance, rehabilitation counseling, social work, education, psychology, or related human service field
• 2 years of supervising experience
• Knowledge of community programs, services, evidence-based practices, and various treatment modalities used to work with youth and their families to navigate child-serving systems
• Ability to actively participate in a fast-paced, multidisciplinary team environment
• Valid driver's license and proof of vehicle insurance
• Strong organizational and time management abilities
• Excellent interpersonal and written communication skills
