This is a Limited Duration position.

The Service Coordinator- Limited Duration position provides Case Management Services for Children and Adults with developmental disabilities. The State Office of Seniors and People with Disabilities contracts with Multnomah County to provide Case Management services. This Service Coordinator position is a KSA (Knowledge, Skills, Abilities) of the Spanish language Case management aims to access, coordinate and assure the delivery of services and supports required by individuals with developmental disabilities. In addition, case Management will assist eligible individuals under their individualized plan in accessing needed medical, social, educational, and other services.

As theService Coordinator-Limited Duration-KSA Spanish, other duties will include:
  • Working primarily with Spanish speaking adults and families
  • Manager a caseload of clients found eligible for DDSD case management services
  • Monitor services/supports via home visits, service provider visit and phone calls
  • Ensure that appropriate services (i.e. financial, medical, training, educational, transportation, and other identified/agreed to services/supports)
  • Develop, facilitate and coordinate resources necessary to provide consumers/families with needed services
  • Prepare and submit routine and special reports.
  • Assess level of functioning and level of supports needed of the individual by completing the TXIX Waiver Forms
  • Counsel and advice consumers/families in regard to placement and care/service plan choice, personal and emotional difficulties, financial planning, and interactional dynamics.
  • Ensure that clients are eligible for services to which they are entitled and receive the support that adequately meets their needs.

Minimum Qualifications/Transferable Skills*:

Education/Training:
  • Ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspectives, backgrounds, and values
  • Education/Training: Associate's degree or equivalent practical experience
  • Experience: Two (2) years of case management experience with a minimum of one year of experience at the full performance/journey case management level
  • Must pass a criminal background check


Experience/Certification:

Multnomah County COVID-19 Vaccination Requirement: To protect the health of the community and employees, Multnomah County requires employees to be fully vaccinated against COVID-19 or have an approved medical or religious exemption as a qualification of employment.   • Language exam is required
  • Must be knowledgeable about and have extensive experience working with the Spanish language community
  • Must speak, write and read fluently in both English and Spanish
