Job Description: Managers will meet the following requirements (with or without reasonable accommodations) for employment with Cascade Residential Care. This position may be eligible for approved overtime.Minimum Qualifications:• Complete satisfactory drug screening, criminal history, abuse history and driver’s license check.• Be a citizen of the United States or legally able to work in the United States• Eighteen years old and high school graduate or equivalent.• Complete agency required trainings including but not limited to Core Competencies, abuse prevention, medication administration, emergency, Individual Support Plan(s), Behavior Support Plan(s), etc.• Be literate and capable of understanding written and oral orders.• Ability to perform intermediate computer operating skills, use common office software, and operate office machinery (fax and copy).• 1 year of experience working as a care provider with individuals with Intellectual and Developmental Disabilities• Regular attendance and ability to perform tasks outlined in this description (with or without a reasonable accommodation) is an essential function of this position. · Ability to become familiar with Oregon Administrative Rules, company policy, forms and systems of the agency/field. Preferred Qualifications: · 2 years or more of experience in a lead position within a group home setting · 3 or more years of experience working as a care provider or otherwise related role · Ability to perform advanced computer operating skills and knowledge of common word processing, spreadsheet, and pdf software. · Extensive knowledge of Oregon Administrative Rules and regulations regarding operations of 24-hour residential facilities for developmentally disabled adults · Previous experience supervising, directing and motivating individuals to work collaboratively in a team environment · Ability to respond to emergencies at all times services are being delivered, being able to arrive at the home within 20 minutes for emergent situations. Essential Physical Requirements:The physical effort typically required in this job includes, but is not limited to, lifting/transferring up to 100 lbs., keying/typing and sitting at computer desk, assisting with cleaning and household duties, applying physical holds (as per Oregon Intervention System OIS instruction) and other duties as needed with or without reasonable accommodation. Professionalism• Demonstrates an active commitment to the Mission Statement and values of Cascade Residential Care.• Motivation to grow individually and professionally and expand the influence of Cascade Residential Care.• Represent Cascade Residential Care professionally at all times, conducting one’s self in a manner that demonstrates a full awareness of role as a Cascade Residential Care representative to co-workers, residents, family members, business partners and the public.• Ability to model Compassion, Accountability, Respect and Enthusiasm to co-workers and new employees and in all contacts with clients and the public.Responsibilities(Job responsibilities are specific duties or tasks an employee is expected to perform) The primary purpose of this position is to manage the operation of a home(s), lead in the development and implementation of Individual Support Plans for each applicable individual and oversee/manage the variety of paperwork and documentation required to ensure that the direct assistance and support individuals receive is effective and consistent and that the home is operated in accordance with the Comprehensive 24-hour Residential Services for Children and Adults with Developmental Disabilities (OAR Chapter 411-325). Position will also provide support to individuals served through assistance with medical appointments, meetings, and training of new staff. Areas of oversight include but are not limited to training of direct support professionals, scheduling, medical, nutrition, housekeeping (maintaining the home/property in a safe and sanitary manner), behavioral management, safety, vocational, health care, finances and other daily activities within the home and/or community. Essential Duties and Responsibilities (estimated percentage of time): 60 % - Works across all three shifts to support team of employees who provide direct care, training and support to individuals. Works as on-call support for individuals’ and employees, may be required to work or cover any of all three shifts in the home. Collaborate on creation of monthly schedule for supervised location(s), approve/manage vacation requests from Direct Support Professional(s) to ensure that needs of supported individuals are met. Assist with management and operation of the schedule, works as secondary on-call support for the schedule and employees and assists to ensure that shifts are covered as needed. Coaches and provides leadership to employees. Assigns work, monitors performance, assists in evaluating performance and assists in supervision. Conducts regular staff meetings to communicate policies/procedures, provide training opportunities, review/implement house rules, resolve house issues and promote teamwork and unity as needed. Ensure that all employees have been trained on all necessary information as required including but not limited to Individual Support Plans, Behavior Support Plans, house specifics etc. 25 % - Oversees that each individual has an accurate and complete Individual Support Plan (ISP) that enhances independence, integration and productivity through assisting with the following: - Ensuring all members of ISP team are notified of ISP meeting; - Work with the individual, specialists, family members, advocates, guardians, employees and management to complete ISP support documents; - Ensuring the completion of the Risk Identification Tool (RIT); - Completion of supports/protocols as directed by the RIT; - Information/input for the Proactive Behavior Support Plan (PBSP); - Development of ISP agenda for the meeting; - Distribution of above documentation to ISP team members prior to the ISP. Implements all ISP documents, protocols, and instructions within appropriate timelines and ensures appropriate training of support documents is completed. Monitors ISP through: - Scheduling and notifying ISP team members of regular meetings as needed; - Reviewing monthly data records for accuracy and completeness, discussing information with other team members as needed; - Regular review of ISP goals/ISP goal tracking to ensure each individual’s needs are being met and action plans are being implemented as outlined; - Assist with updating the ISP and/or support documents as needed. Maintain all individuals’ records in a confidential manner. Maintain all documentation (i.e. physician’s orders, current medical conditions, physician visits, personal property records, Medication Administration Record (MAR), financial forms etc.), ensuring accurate, current and complete and in compliance with OAR and company policy. Works with physicians, dentists, health care consultants, health care representatives and other service providers as needed to support individuals’ health and well-being. Schedules appointments and arranges for the individual to keep appointment. Assist individuals in attending appointments and/or meetings as needed. Complete documentation associated with appointments and/or meetings as required. Researches and accesses community resources and services as needed. Reviews employee documentation of incident reports in a timely manner, reviewing incident and forwarding document to management, addresses concerns with management and/or employees as needed. Maintains reports. Files and maintains records appropriately and in accordance with company policy and OAR’s. Review the MAR and medications at least weekly for accuracy and completeness and to ensure all medical protocols are followed. Reviews employee documentation of financial records and available funds at least weekly for accuracy and completeness and to ensure financial plans are followed. Collects, reviews and submits individuals’ receipts with financial forms monthly. Manages petty cash in the home ensuring necessary funds are available and used in accordance with company guidelines/policy. Collects, receives and submits petty cash receipts and forms monthly. 10% - Reads, understands and implements CASCADE RESIDENTIAL CARE policies and OARs that address 24-hour operation of residential homes. Oversee meals prepared in the home, ensuring that a variety of healthy food is available and meets individual dietary needs. Works with employees to ensure physician ordered diets are followed and documented on the menus. Oversee and assign household duties to maintain a clean, sanitary, safe and inviting environment, assist in coordinating repairs as needed. Schedules monthly safety check, reviews records, corrects issues when arise and addresses issues with management and/or employees as needed. Maintains reports. Schedules regular fire drills, reviews records, addresses issues with management and/or employees as needed. Maintains reports. Ensures assigned vehicles are maintained, schedules regular vehicle safety checks, reviews records, corrects issues when they arise and addresses issues with management and/or employees as needed. Maintains reports. 5 % - Attend meetings (management meetings, house meetings, ISP meetings etc.) as scheduled and trainings as assigned. Works on special assignments/projects and serves on work teams/committees. Perform all other duties as needed/assigned.
