WE ARE HIRING Case Managers to work in Disability Services.
Job Responsibilities
- Provides direct case management services to individual consumers and their families based upon thorough assessment of the consumer's situation, wants, and needs.
- Perform initial intakes and routine assessments to determine consumer's interests,
- Strengths, needs and evaluate progress.
- Develop service plans with the consumer, family, caretaker, or other parties.
- Creatively seeks and develops needed resources to meet consumer needs.
- Meets with clients based on required meeting schedule as established by the state, division and/or department.
- Effectively responds to crisis or emergency situations with supervisory consultation.
- Organizes and utilizes time and resources effectively.
Experience of the following is preferred.
- One year of exp with ID/DD population
- Conducting interviews and meeting
- Developing service plans
- Electronic health records.
- Experience with Medicaid waiver
Special Credentials
- QDDP (Qualified Developmental Disabilities Professional) Certified or Eligible
Pay Rate $22.87
Job Requirements:
Minimum Qualifications:
Requires a four year degree in a human services field and one year of experience working with persons with intellectual disability.
Any combination of education (above the high school level) and/or experience equivalent to five years in fields utilizing the required knowledge, skills and abilities working with persons with intellectual disability.
Possess a valid Driver's License and acceptable Driving record to meet job requirements.
