Job Title: Social Work/Case Manager (MSW)

Job Summary

Responsible for the direct social work case management service to the participants of the PACE of Guilford and Rockingham Counties program. Provides screening, psychosocial assessment, care plan implementation and appropriate interventions including but not limited to counseling, referrals, and education.
